Lead qualification forms that take the payment and book the call

A lead qualification form scores a few answers and sends each person to the right next step: a call, a payment, or something free while they are not ready. Which questions to ask, when to ask for money instead of a call, and how Webinly Forms and booking pages do both.

How to qualify leads with a form

Qualifying with a form means deciding before the call, not on it: answers add points, the total sets a band, and each band ends somewhere different.

BandWhoWhere the form sends them
ReadyA fit, with the timing and the budget to start.Book a call, or take the payment.
WarmA fit, but not now.A replay, your next webinar or a free resource, then follow-up.
Not a fitSomeone you cannot help, or not yet.An honest thank-you, not a call that goes nowhere.

Score facts a person can answer honestly (timing, situation, what they have tried), not how keen they sound. And never send the lowest band to a dead end: someone not ready today can only come back later if they are on your list.

Which questions to ask

Five to eight is enough. Each question should change the score or give you something to read before the call; if it does neither, cut it.

QuestionWhat it tells youHow to score it
Name and email (and phone, if you will call)Who they are, and where the answers go.Not scored, but without an email there is nobody to follow up.
Which of these best describes you?Whether you can help them at all.Points per choice, zero for the ones you cannot serve.
What have you already tried? How far along they are.Points per choice, or unscored and read.
When do you want to start?Timing. Most for this month, fewer for later, none for just researching.
Have you set aside a budget for this?Whether they can act on a yes.Points on yes, or per range.
In a sentence or two, what do you want to change?Their own words, for the call and for your copy. Not scored. Read it before the call.

Open with easy questions about their situation and put budget near the end. Contact details can go first, so you keep the lead if they stop, or last, so fewer stop early. In Webinly you can ask early and add a partial-submit point right after: anyone who quits later is still saved as a contact.

A coaching intake form is the same form with the top band sent straight to your booking page.

When to ask for payment, and when to book a call

A form can end in a payment, a call, or a payment then a call. Pick by how much trust the sale needs.

What you sellEnd the form withWhy
A fixed-price product the buyer understands: a toolkit, an audit.Payment in fullA call adds a step nobody needed.
A paid session: a strategy call, a review.Payment, then the booking pageThey pay for the time, then pick it.
An offer that needs a conversation before anyone buys.A call, no paymentMoney before a conversation loses buyers who would have said yes on the call.
Calls where a no-show costs you a real slot.A small deposit, then the booking pageA small commitment before the slot is taken. Say if it counts toward the price.

Whichever you pick, say so before the last question: a surprise payment screen reads as a bait and switch. Selling on a webinar? Put the form after it for people who need a call; in-room offers cover the ones ready to buy during the session.

How it works in Webinly

Webinly Forms is a form builder with Stripe payments and call booking in its endings, on every plan including Starter. Each form has its own link. You can embed it on your site inline, or open it from a button or link in its own pop-up window.

Four modes

Universal and Lead qualification are for forms with a single ending. Knowledge quiz and Match quiz allow several endings. Knowledge quiz scores the form, by correct answers or by the points you set, and sends each score range to its own ending, so it is the mode for a qualifying form with bands like the one above. Match quiz points each answer at an ending and the ending with the most picks wins, which is how a quiz funnel works: every result is an ending, and every ending can send people somewhere different.

The questions

One question per screen. The types you can add: a contact block (name, email, phone, company, address and website in one), email, phone, address, website, multiple choice, dropdown, picture choice, a confirm checkbox, yes or no, terms and conditions, rating, opinion scale, NPS, ranking, matrix, short text, long text, statement, a recorded video or audio answer, number, date, file upload, signature and a partial-submit point. A form written in Arabic runs right to left.

Logic and scoring

A rule can jump to another question, end the form, or send the person straight to a chosen ending. Conditions combine, can check any earlier answer, and can read a parameter in the link, so ad and email visitors can take different paths through one form. Scoring gives points per choice on multiple choice, dropdown and picture choice questions, and points for yes or no. Webinly recalculates the total on the server, so nobody can edit the page to score into a better ending.

Where each ending sends them

In Knowledge quiz mode, endings carry a score range, checked in the priority order you set; one with no range catches everyone else. In Match quiz mode the answer tally picks the ending, and a range only applies when no mapped answer was picked. Each ending can tag the contact and does one of these things.

  • Show a thank-you message.

Written per ending, so each band reads its own.

  • Book a call.

Your booking page opens with their name and email filled in.

  • Take a payment.

Checkout on your own connected payment account, such as Stripe or Tap, for an amount you set or a saved offer's current price. Once it clears: a thank-you, your booking page, a webinar's own page (where they sign up to watch) or a link.

  • Send them to a webinar replay.

The pitch they missed, for the warm band.

  • Register them for a webinar.

With the email they just gave.

  • Open a link you choose.

It can carry their name, email and incoming link parameters.

When the charge runs through Stripe, the money lands in your own Stripe account, with the usual Webinly checkout fee: 5% on Starter, 2% on Pro, none on Scale.

What lands on the contact

A form with an email question, or a contact block with Map to contacts switched on, creates or updates a contact. The response is saved on it, mapped answers update its fields (custom fields included), and the visit’s UTM source is kept. The submission shows on the contact’s timeline next to their webinar activity and live lead scores, and it can start an automation. You get an email per response by default; a confirmation to the respondent is optional.

Booking pages: the call at the end of the form

A booking page is one kind of call: its length (or a few to choose from), where it happens (a phone number, an address or a meeting link) and when it can be booked: weekly hours in your time zone, date overrides for days off, a minimum notice, how far ahead, and buffers around each call.

Connect Google Calendar and choose which calendars count as busy and which one receives bookings. Busy times disappear from the page, each booking is written to your calendar, and a reschedule or cancellation updates it there.

The visitor sees times in their own time zone, detected from the browser and changeable. Arriving from a form, their name and email are filled in, so they do not type them again. They get a confirmation, reminders one day and one hour before, and a link to reschedule or cancel. Each email’s wording is editable per booking page.

The booking lands on the contact record, and “they book a call” is an automation trigger. Booking pages also work on their own: share the link, or drop the Calendar block into a page built in the page builder.

What it does not do

Calendar sync is Google Calendar only. Outlook and Apple calendars cannot be connected. With no calendar connected, bookings still happen, but they are not checked against your other meetings and do not appear in your diary.

Payment and booking come after the last question. They are what an ending does, not fields inside the form. A form payment sells one fixed-price item, one charge at a time: no subscriptions, no instalments, no quantities and no stock tracking.

Several endings need a quiz mode. Universal and Lead qualification forms have one ending. To send different scores to different places, build in Knowledge quiz mode, or in Match quiz mode for a quiz funnel. Pick the mode before you add endings.

No email, no contact. Without an email question or a contact block with Map to contacts switched on, the form creates no contact of its own. Responses are saved and your per-response email still arrives, but the form tags nobody, sends the person no confirmation, and its automations cannot email or tag them. A webinar or replay ending still signs them up with the email typed into the contact block.

A paid ending never turns free. If checkout cannot open, say because Stripe is not connected, the person is told and you are alerted. A booking page that was switched off is flagged on the response instead, and the person sees the ending’s thank-you screen.

Set one up in Webinly

  1. 1. Connect Stripe and Google Calendar

Stripe for payments, Google Calendar for booking.

  2. 2. Create the booking page

In Webinly Scheduling, create an event type (its public page is your booking page): length, weekly hours, notice, buffers and where the call happens.

  3. 3. Build the form

Blank or from a template, in Knowledge quiz mode (one of the two modes that allow several endings, and the one that routes by score), with points on each scored answer. Points you set replace the correct-answer count.

  4. 4. Add one ending per band

Each with a score range and a destination.

  5. 5. Fill it in three times

Once as each band. Check each ending lands where you meant, the response shows the right answers and score, and the contact has the right tags. For the paying band, stop once the checkout page opens rather than paying: with test mode switched on (Settings > Payments), a Stripe form payment is refused on purpose and you get an alert instead of a checkout.

Common questions

What is a lead qualification form?
A short form that asks about fit, timing and budget, scores the answers, and sends each person to a matching next step: a call or a payment for the ready ones, something free for the rest.
How many questions should a lead qualification form have?
Usually five to eight, one per screen: contact details, two or three questions on fit and timing, a budget question near the end, and one open question you read before the call.
Can a form take a deposit and then book the call?
Yes. Set the ending to take the deposit and send people on to your booking page once it clears. They pay at checkout, then pick a time with their name and email filled in.
Which calendars does booking sync with?
Google Calendar. You choose which of your Google calendars count as busy and which one receives bookings. Outlook and Apple calendars cannot be connected.
Can I build a quiz funnel with Webinly Forms?
Yes. Match quiz mode points each answer at an ending, and the ending with the most picks wins. Each result can book a call, take a payment, open a replay or register the person for a webinar.
