Order Form Template With Payment (2026): Fields and Copy

· 10 min read · Webinly team

An order form has one job: turn a clear yes into a paid order with nothing left to chase. Most of the ones people copy ask for the card before the total is on screen, end on a blank 'submitted' page, and keep no record of who bought. Each of those loses a sale.

Below is the whole form as plain text to paste, then the three ways the same form takes the money in Webinly, on Stripe or Tap, and what happens on its own after someone orders.

What should an order form include?

An order form should collect who is ordering (name, email, and a phone number if you deliver by phone or WhatsApp), what they are ordering and how many, any options that change what you deliver, delivery or access details, an agreement to your terms and refund policy, and the payment. Show the total before the first card field, and end on a confirmation that says what arrives, where and when. Every other field costs you a few buyers.

Easy questions first, the money last, the price known from the first product screen and repeated on the pay button. The template below is for a digital product in three versions and uses only question types that Webinly Forms offers, so it builds as written.

The order form template, written out

One question per screen, help text under the field, the price on the first product screen. The prices are placeholders.

  • Screen 1, Your details. Full name. Help: As on your receipt. Email. Help: Receipt and access go here, so check the spelling. Phone, with country code. Help: Only used about this order.
  • Screen 2, What are you ordering? Pick one: The workbook, $49. The full program, $297. The program plus a 1:1 review, $396. Help: The price you pick is the price you pay.
  • Screen 3, How many? Only if you sell several of the same thing. Number, 1 to 10. Help: One licence per person.
  • Screen 4, Options. Which language do you want the materials in? Dropdown: English, Arabic. Help: You can switch later.
  • Screen 5, Delivery or access. Posted: Delivery address, an address block. Digital: Anything we should know before we send access? Long text, optional.
  • Screen 6, Agreement. Checkbox, required: I have read the refund policy and agree to the terms of sale. Help: The policy is linked here. Button: Review my order.
  • Screen 7, Review and pay. Summary: The full program, $297, one payment, charged today. Then the card fields. Button: Pay $297. Under it: Secure payment. Receipt by email.
  • Confirmation: Thank you, {first name}. Your order for the full program is confirmed. A receipt has gone to {email}; your access details follow in the same inbox. Wrong email? Reply to the receipt.

Field by field: required or not, and why it is there

If the third column would be empty for a field you are about to add, leave it out.

FieldRequired?Why it is there
Full nameYesOn the receipt and the contact record.
EmailYesReceipt and access go here; it matches the order to an existing contact.
Phone, with country codeOnly if you call or message buyersA failed delivery or a wrong email. Useless abroad without the code.
What are you ordering?YesThe product and its price, on screen before any card field.
How many?Only for several of the same thingSeats or copies. On a one-item order it invites a mistake.
OptionsOnly when it changes what you deliverLanguage, size, format, start date. Price changes belong in the product choice.

NotesNoA different receipt email, a colleague to copy, a preferred start.

PaymentYes, lastThe card fields, after a line that repeats the product and the total.

Three ways to take the money in Webinly

Webinly puts forms, payment links and a funnel builder on one account, so an order form can be built three ways. All three charge on a Stripe or Tap account you connect in Settings, Payments, and the money lands there, not in Webinly.

  • A form with a payment ending. On every plan including Starter, the ending charges an amount you set or a saved offer's current price on your connected Stripe or Tap account, then opens a thank-you screen, your booking page, a webinar's own page or a link. It sells one fixed-price item, one charge at a time: no subscriptions, instalments, quantities or stock tracking. If the checkout cannot open, the person is told and you are alerted; a paid ending never turns free.
  • A payment link. A web address that opens a checkout for one offer at one price. Each link sells the main offer of one of your free webinars, can hold a sale price for one buyer for 1 hour to 30 days, can be locked to one email, and lets one buyer pay for up to 20 seats in one card payment. Every plan, once a processor is connected.
  • A checkout page in the funnel builder. It asks for name and email, optional phone and country, and a terms box. An order bump shows as a checkbox that adds its price to the same payment, and after a Stripe card payment a one-click upsell can follow, then a downsell. No coupon codes: the buyer pays the offer's price in one payment. Funnels is off by default and switched on per account on request.

What happens after someone orders

The part most order forms skip, and the part Webinly does on its own.

  • After a form payment: a form with an email question, or a contact block with Map to contacts switched on, creates or updates a contact. The response sits on its timeline next to their webinar activity and scores, mapped answers update its fields, and the submission can start an automation. You get an email per response; a confirmation to the respondent is optional. No email question, no contact.
  • After a payment link: the buyer is in your contacts before they pay, one profile per email in the customer data platform; anyone who stops lands in the Checkout abandoned segment. Once paid, the contact is tagged purchased, automations that start on Pays you begin, and what the offer is set to deliver (picked on automated webinars; a course can be one) is handed over the moment they pay.
  • After a funnel checkout: a thank-you page with your content above the list of what they bought and the receipt, the buyer in your contacts tagged with the funnel, and a Bought something automation if you built one.

Can buyers pay with mada, KNET or Apple Pay?

Yes, where Tap or a KNET gateway handles the price. Webinly runs Stripe and Tap side by side, so Gulf cards, KNET and Tabby instalments work on a checkout, with prices in KWD, SAR or AED. On a payment link priced in KWD or SAR, Tap takes Visa, Mastercard, mada and American Express; KNET appears on KWD offers when a KNET gateway handles them; Pay in 4 appears on AED, SAR and KWD offers beside the card or KNET option, never alone. On a paid webinar, Tap's own payment page can offer KNET, mada, Benefit or Apple Pay, depending on your Tap account.

Two limits: funnel offers are priced in USD, EUR, GBP, CAD or AUD, and a one-click upsell charges only a card saved by a Stripe payment, so a buyer who paid by Tap or KNET skips it.

How to create the order form in Webinly, step by step

Six steps, from a blank account to a form that charges.

  1. 1Connect a payment account in Settings, Payments: press Connect Stripe, or paste your Tap secret and public keys and press Connect Tap. A payment ending cannot charge without one.
  2. 2Create the form, blank or from a template, and pick the mode before you add endings: Universal for one product at one price, Match quiz for several prices, each choice pointing at its own ending.
  3. 3Add the questions, one per screen: a contact block with Map to contacts on, a multiple choice question with the price in each option, a dropdown for options, long text for notes, and a terms and conditions question.
  4. 4Set the ending to Take a payment: type the amount or pick a saved offer, choose what opens once it clears (a thank-you, your booking page, a webinar's own page or a link), and tag the contact.
  5. 5Publish, then share the form's own link or embed it inline or in a pop-up window. A form written in Arabic runs right to left.
  6. 6Fill it in yourself and stop once the checkout page opens: with test mode on in Settings, Payments, a Stripe form payment is refused on purpose and you get an alert. Switch test mode off before the first real buyer.

Order form mistakes that lose the sale

Three mistakes show up on most order forms, and a fourth is specific to payment forms.

On price, the pricing page, read on 25 October 2026, is the source: Starter is $39 a month and includes Forms with payment endings, payment links and checkout in the room; Pro is $159 and Scale $349. The price is per account, not per seat, and your processor charges its own fee.

  • The card before the total. Put the price on the product screen and again on the pay button; a card field before the number reads as a trap.
  • No confirmation. Name the product, say where the receipt went and what arrives next. In Webinly a form ending shows the thank-you you wrote once the payment clears; a confirmation email to the respondent is a setting you switch on.
  • No record of who ordered. Every Webinly checkout puts the buyer in your contacts; on a form that needs an email question or a contact block with Map to contacts on.
  • A quantity box on a one-item payment. A form ending sells one item per charge, so a How many field it cannot honour breaks at checkout. For several seats, send a payment link: up to 20 in one card payment.

Frequently asked

What should an order form include?

Who is ordering (name, email, phone if you will call them), what they are ordering and how many, options that change what you deliver, delivery or access details, an agreement checkbox for the terms and refund policy, and the payment last, after a line showing the total, then a confirmation saying what arrives next.

Can an order form take payment?

Yes. In Webinly a form ending takes a payment on your connected Stripe or Tap account, for an amount you set, on every plan including Starter. It sells one fixed-price item per charge, no subscriptions, instalments or quantities, then opens a thank-you, a booking page, a webinar page or a link.

How do I make an online order form without code?

Use a form builder that charges in its ending. In Webinly: connect Stripe or Tap, create a form blank or from a template, add a contact block, a product choice with prices and a terms checkbox, then set the ending to Take a payment. Publish, then share the link or embed it.

Can people pay with mada or KNET?

Yes, where Tap or a KNET gateway handles the price. Webinly runs Stripe and Tap together, so Gulf cards, KNET and Tabby instalments work on a checkout with prices in KWD, SAR or AED. On a payment link, Tap takes cards, mada included, and KNET appears on KWD offers when a KNET gateway handles them.

What happens after someone orders?

The buyer becomes a contact. A form with an email question saves the response on the contact's timeline. A payment link saves the buyer before they pay, tags them purchased and hands over what the offer delivers. A funnel shows a thank-you page with the receipt and tags the contact with the funnel.

Is this included on the $39 plan?

Yes for the form and the link. Starter, $39 a month, includes Forms with payment endings on Stripe or Tap and payment links. The price is per account, not per seat. Funnels is switched on per account and needs an active plan. A first plan is $1 today and the plan price 7 days later.

Paste the template into Webinly Forms, connect Stripe or Tap, and let the ending take the payment and keep the record: your first plan is $1 today and the plan price 7 days later, cancel any time.
